Forgot Intel Edison log-in info

Hi everyone. I configured my edison and set-up a wi-fi connection. However, when I try to run putty, I see a screen displaying this:

I forgot my log-in information, so I can't run the reboot ota from here. I also can't run 'run do_ota' in putty after resetting my edison board because I get the 'Unable to read file ota_update.scr" line. I've tried flashing my edison  board 6 times to no avail and even tried using the Sw1UI3 and SW1UI4 methods. IAny help would be greatly appreciated.
